1. Requests

• if pt/family requests, it’s a professional obligation to put in the order.
  • if they request once, “Do you want to put in the order?”
• Considerations:
  • Imminent?
  • Where to they want MAID (home, MAID house, hospital)
    • Matters: who does assessment
    • Sometimes will necessitate different teams doing it
• If a patient is alluding to symptoms, MAID or suicide, physicians may bring it up. “We have medical assistance in dying, would you be interested in learning more?”
  • it’s a choice if we bring it up or not

1. Requirements for MAiD:

• OHIP

• 18 years old

• Must be able to consent
• Grievous + irremediable + severe, life threatening illness (no need for immediately life threatening)
• Voluntary

Two Tracks Stream 1: Death imminent

• 1 assessor
• no waiting period
• waiver of final consent (if there is a risk of losing capacity to consent, someone else can consent.) If you do this, you must pick a day.

Stream 2

• 2 assessors (one of the assessors does not need to be the MAID provider)
• 90 day reflection period
